The PCMH Clinic Quality Manager Solution Center
The PCMH Solution Center provides any clinic with the needed platform to accelerate your strategy and plan to migrate to a Patient Centered Medical Home (PCMH). It tracks patient treatment plans and processes in the clinic (example: Diabetes, Cancer Screening. Etc.) and provides electronic forms to collect data where it cannot be electronically gathered at present, provides a library for standard forms to be used by all clinics, provides options to track volunteer hours, cost savings, appointment data, etc. Collects data for upload to the data warehouse, provides forms and workflow for Quality Measures information not attainable by electronic means – updates a quality database for upload to the data warehouse. Has real-time search capabilities and email/SMS alert system when a Care Measure is out of range or a patient event is recognized.
