Technology Medical Partners Solution Selected by Grace Medical Home

 

New TMP and Microsoft solution extends any EMR to provide clinical quality management, reporting and improved patient outcomes for healthcare providers

Cincinnati – June 8, 2010 – Grace Medical Home (Grace) of Orlando, Fla., which offers ongoing primary care needs of prevention, wellness and sick care to the low income, underserved of Central Florida, is leading the way in next generation community health care. By linking its patient-centric approach, Electronic Medical Records (EMRs) and the new Clinical Quality Management (CQM) solution from Technology Medical Partners (TMP), Grace is providing a fuller picture of the health care of those served at the Medical Home while streamlining the reporting process and allowing doctors and nurses to spend more time with patients.

 

Grace’s selection  and implementation of Technology Medical Partners’ (TMP) Clinical Quality Management (CQM) solution will tie reporting, tracking, workflows and data management directly to Grace’s EMR implementation, streamlining compliance, quality and community reporting requirements.    

Grace, which serves as a missing link in Central Florida’s healthcare system, has been a model of community engagement, with support and investment from regional and national organizations, including Orlando Regional Healthcare, Walt Disney World Company, Florida Hospital, Microsoft and Technology Medical Partners; amongst a long list of other contributors.

Marvin Hardy, M.D., Grace Medical Director, states, “The idea of a medical practice for the underserved has been on my heart and mind since medical school. Those who are uninsured and low income, yet created in God’s image, are not receiving the same quality of healthcare as others of greater means.  With the help of TMP and its CQM Solution, we can focus on our mission as efficiently as possible.”

 

The mission of Grace is to provide holistic quality care to an underserved population. Reporting outcomes and analysis of patients’ continuum of care is essential to ensuring the “Medical Home” model works for growing U.S. communities. 

“Caregivers want to spend more time providing care and less time filling out forms or managing spreadsheets,” said Stephanie Garris, Grace’s Executive Director.  “This new approach allows us to not only remove or reduce the doctors’ and nurses’ administrative tasks; it will also show us new ways to improve performance, based on all the data we’re now extracting.”

There are significant community needs for quality reporting and other dashboard capabilities that can be provided through the CQM Solution.    With the Clinical Quality Manager, Grace has the ability to:

·               Follow and report on evidence-based medicine processes;

·               Track and report on long-term patient education programs, patient care and outcomes; and

·               Automate forms, documents, schedules, demographics and other staff functions.

 

James Dixon, Managing Partner at Technology Medical Partners says, “This system takes an essential step in addressing the fact that every clinic, hospital and physician will need a solution to report their quality of care core measures to CMS (Medicare), Medicaid, state regulatory agencies, physicians, board members and patients they serve.” 

TMP meets this need with the CQM solution built on a standard Microsoft Platform, using a SharePoint-based secure portal that improves the access to EMR data.   The solution provides:

·               Workflow efficiency of forms and data entry;

·               Report management and Analytics;

·               Data abstraction and aggregation (via dashboards) from any EMR;

·               Reusable quality database; and

·               Management of the many reports, forms and activities at the clinic.

 

The CQM solution also addresses CMS required reporting that relieves nurses (that are in short supply) from arduous and time consuming data abstraction from various sources and helps them focus on patient care. Ultimately, the CQM solution will assist clinicians to improve the quality of care of their patients in real time.
